How to Register

Registration of New Patients


All new patients registered with the practice will be asked to complete a medical questionnaire and new patient registration form.
An appointment will be made with the HCA for a new patient examination which enables us to carry out a full health screening, i.e. blood pressure, weight, height, smoking status, past and present medical history, and family history.

When your paperwork is complete you can:

• Bring it in to the surgery
• Post it to the surgery
• E-mail it to the surgery, west.street@gp-k84030.nhs.uk
